2 Application Form

One China visa application form per applicant completed on-line, printed and signed. Signature must be original on each visa application. Improperly completed forms may delay the processing of your visa.

3 Photograph

You must submit one passport photograph taken by a passport photographer. Digital, homemade, and non-passport style photographs are not acceptable.

4 Non-US Citizens

Applicants who are not Citizens of USA, but hold long term US visa or a Green Card should submit your actual Green Card or U.S. visa. Non-US citizens must also fill out a supplementary application form

5 Letter of Admission

Send the original and a photocopy of your Admission Notice from a Chinese school.

6 Application for Study in China 

Aka JW202 or JW201 form. Please send the original and a photocopy of the form.

7 Physical Examination

Please download the Physical Examination Record for Foreigner , have your physician fill it out and send it with a passport type picture attached.

8 Complete Online Order Form

You must complete the online order form. You will be asked to input your personal information, order request, and payment details. After inputting your data, click on "Submit Order " which will create your completed order form, then print it out.
